add an additional note field in the 1099 payments tab

Some vendor names are not clear as to whether they are an LLC or Corp until we get their W-9.  We should have an additional field in the 1099 payments tab where we can add notes on what boxes were checked on the W-9 to help us double check that the always/optional/never is set up correctly when reviewing for 1099s.
